Commercial Licensing Forms

1. Application forms may be subject to amendment therefore it is advisable that forms only be down-loaded from the website at the current time of their use and not copied off in quantity.
   
   
2. Once down loaded the text/ format of the form should not be modified. Modification to the text of the form could render the approved form incompetent.
   
   
3. When completing application forms please read the instructions attached to the forms carefully. Failure to complete all sections of the form, could render the application incompetent and lead to it being returned to the applicant.
   
   
4. Under the provisions of the Stamp Act 1921, stamp duty is payable with respect to the transaction the subject of T1, T2, T3 and V2 application forms. Please refer to ‘Circular No.73' provided by the Office of State Revenue for further details regarding your obligations under that Act, and for the requirements that must be satisfied before the Executive Director can receive these application forms. 5. Once completed the original form can be forwarded to Head Office (168-170 St Georges Tce Perth 6000) with the nominated fee (see Schedule 1 of Fish Resources Management Regulations 1995) enclosed subject to 4 above being complied with.
